Rapidocs Intake Questionnaires

INTAKE QUESTIONNAIRES

 

If you want to use your own legal documents and legal forms and don't have time to automate the,. another choice that you have is to create a Rapidocs Intake Questionnaire. The Questionnaire can be built from Intake Questionnaires that you already use in your office. It can be processed through the shopping cart, just like any other automated document template. The Questionnaire will export an Answer File that you can use to assemble a document using what ever desk-top document assembly tool you already use. It is less efficient to use this work flow, as distinguished from using an automated template to create a first draft of your document, but for those lawyers who insist on using their own legal form content, or have a document that they want to use but is not yet in our automated inventory, this is an alterantive.

 DirectLaw is happy to assist you in creating an intake questionnaire using Rapidocs, our document automation proprietary software.  We will automate your questionnaires for a fee of $50.00 a page.Some pre-planning on your part is required to make this process quick and efficient.

 

Step 1 –Purpose of intake questionnaire

Determine why you are creating an intake questionnaire.  Is it to collect general client data or is it to collect data to complete a legal document (such as a will, power of attorney, lease agreement, etc.), or both?

Step 2 – Itemize what information you need

Determine ALL the questions you will need to ask in order to collect a complete set of answers needed for your relationship with the client and/or the legal document.

Step 3 – Provide DirectLaw a “question outline”

Refer to the second page of this document.  It provides you with a starting outline for your intake questionnaire.  Complete this page and feel free to edit some of the standard questions we have already created.   You must draft your own questions and help text (or explanations). 

Step 4 – Finalizing your intake questionnaire

This service of creating an interactive intake questionnaire includes the following:

·        We receive your intake questionnaire “question outline”.  (You must supply the content.  We will not draft question prompts, explanations, etc.  We will build the questionnaire as you supply the information.)

·        We create an interactive intake questionnaire containing your content utilizing Rapidocs.

·        You will review the draft of the intake questionnaire.  You will have the opportunity to test the questionnaire via a web link and password we provide to you.

·        At this point, you will either approve the intake questionnaire or provide us one round of edits.

·        We will update the intake questionnaire and provide you another online test of the questionnaire for your final approval, if applicable.

·        Your intake questionnaire is completed and ready for upload to your DirectLaw platform.

·        If your questionnaire exceeds one page, you will be invoiced $50 for each additional page beyond page one, including any partial pages.  (We use a 10 pt font and a page with 1 inch margins.)

·        Upon final project payment, if any, your intake questionnaire will be uploaded to your DirectLaw platform.

·        Subsequent edits to your intake questionnaire will be subject to a project review and quote.  Our professional document automation services are subject to our regular rate of $107/hour. 

 

Note:  For the intake questionnaire project to remain cost effective, you must carefully plan Steps 2 and 3 listed above.  If the review of the first draft returns significant edits or additions that were not included in your initial “question outline”, we may at our discretion invoice you for professional document automation services at our regular rate of $107/hour. 

INTAKE QUESTIONNAIRE

Complete the following outline.  Some standard questions have been created to illustrate how you should organize your questions, as well as show how to provide the prompt (the actual question your client will see) and explanation (which will appear as a popup “help” box next to the question). 

The standard questions and the related prompts and explanations below can be edited to accommodate your style and needs.  Feel free to completely or partially delete the questions/prompts/explanations below, and add your own questions/prompts/explanations.

The end result will be a Rapidocs template that will be uploaded to your platform.  In the Customize Store tab, you will select, price (if applicable) and offer it on your DirectLaw platform.  When a client selects the questionnaire, they will complete it via a web browser and submit the finalized questionnaire to you.  In the Submitted Docs tab of your platform, access this completed questionnaire which can be exported to Word.  You will have a document that appears in table format, with questions listed on the left and answers on the right.  At this point, you have the raw answers that you need to then utilize in your legal document(s) as appropriate.

You will have to complete the following form for our development team in detail. We will send you this form for completion, once you have signed up for this service, and made your initial payment. The following form is for information only to help you with your pre-planning process.
